 INFOhio Sirsi K12 Go-Live Form

Complete this form when a school district has made the transition to the Sirsi K12 Application (or has scheduled a firm "go-live" date).  The information collected via this form will be used by INFOhio Central, INFOhio Technical Services, and Sirsi ClientCare in an effort to monitor the overall progress of INFOhio's transition from MultiLIS to the Sirsi K12 product.
